Many health benefits are associated with regular exercise, and the completion of PAR-Q is a sensible first step to take if you are planning to increase the amount of physical activity in your life. For most people, physical activity should not pose any problems or hazard. PAR-Q has been designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advise concerning the type of activity most suitable for them. I understand that the personal training I receive is provided for the purpose of exercise instruction and guidance. I further understand that personal trainers are not qualified to perform, diagnose, prescribe or treat any physical or mental illness, and as such nothing discussed during the course of a session should be considered as such. I understand that I should seek the advice of a doctor or other medical professional for any nutritional or mental health concerns that I am aware of. I confirm that I have stated all known medical conditions and answered all questions honestly. I agree to keep the personal trainer up to date with any changes in my medical profile, and understand that there shall not be liability on the personal trainer’s part should I forget to do so.

Client information - Waiver of Liability

I recognize that the programme I have signed up to with MEE-FIT may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training and various other fitness activities. I hereby confirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent me or limit my participation in this exercise programme. In consideration of my participation in this programme, I hereby release MEE-FIT from any claims, demands and causes of action as a result of my voluntary participation of the provided personal training services and training. I fully understand that I may injure myself as a result of my enrolment and subsequent participation in this programme and I hereby release MEE-FIT from any liability now or in the future for conditions that I may obtain. These conditions may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, knee injuries, foot injuries, or any other illness or soreness that I may incur, including death.

Client information - Session Policy

The most important factor in improving health, fitness and achieving results is consistency. Training regularly and turning up for each session is just as important as the quality of the workout. Personal training guarantees you will receive quality instruction and professional service. Our training session is valuable and we ask that you respect this by giving us notice of 12 hours prior to any designated session if you are going to be unable to attend, failure to do so will result in sessions still being counted as one of your allotted sessions. You will be coached through every session by a MEE-FIT coach and the coach may change from session to session depending on booking availability. No refunds will be given for missed sessions or cancellations of programmes. We understand that there are times that you may not be able to participate in the session due to an injury. But this does always not mean you should not train at all. As long as you give us 24 hours notice we can plan an alternative session for you to take into consideration your injury.
